Lakeview Health Scholarship
Scholarship Sponsored by Lakeview Health
The purpose of our scholarship is to help support an undergraduate or graduate student looking to further their academic studies and achievements. This scholarship will grant one recipient a one-time reward of $3,000 that will be submitted to their chosen university for the fall semester.
Scholarship Qualifications
Scholarship applicant must:
A) Be 18 years of age or older
B) An American citizen
C) Retain a current grade point average of at least 2.5.
D) Be currently enrolled in or accepted to a four-year or two-year college or university.
E) Meet the scholarship regulations and include the submission requirements listed below.
How to Apply for Our Scholarship
To apply for the Lakeview Health Scholarship, eligible students must complete the form below by June 14, 2021. Students may choose to answer one of the two essay topics listed below, the essay must be 1000-words and 100% unique, written in the voice of the applicant.
Topics to choose from:
A) Provide 3 steps that students like you can implement to reduce the role that drugs and alcohol have in your community.
B) How has your life changed for the better since you or someone close to you entered recovery?
Scholarship Award Determination & Delegation
A) Lakeview Health will determine the scholarship award winner by essay originality, the impact of the scholarship, grammar, word choice/selection, fulfillment of word length and 100% unique content confirmation.
B) Lakeview Health will announce the 2021 scholarship award winner on August 2, 2021.
C) Lakeview Health will then send the $3000 via cashier's check to the university, college or community college of the winner's choice.
Scholarship Regulations
A) Essay submissions must be 100% unique, written only by the applicant. Plagiarism of any form will result in immediate disqualification.
B) Essay submissions that are sent after the deadline of July 1, 2021, will be rejected.
C) Essay submissions that fail to include any of the required details listed or don't meet the specifications for eligibility will be rejected.
D) The scholarship award can be revoked at any time if Lakeview Health determines that the winning student broke any rules.
E) Applicants may not contact Lakeview Health to request an update on their essay submission status.
Applicants must be willing to have their likeness and essay published on the company websites and social media channels.
